Cancellations
Out of courtesy, please give at least 24 hours notice of any cancellation.
Appointments canceled with less than 24 hours notice may be charged the full session fee.
Late Arrivals
If you arrive late, your session may
be shortened in order to accommodate others whose appointments follow
yours. Depending upon how late you arrive, Brian will then determine if
there is enough time remaining to start a treatment. Regardless of the
length of the treatment actually given, you will be responsible for the “full” session. Out of respect and consideration to your therapist and other clients, please plan accordingly and be on time.
Payments
Payment may be made using cash, check, or gift certificate.
Gift certificates may be purchased in the office, or over the phone.
Insurance
At this time Brian does not handle insurance forms/claims. If needed, he can provide you with receipts for you to submit to your insurance company for possible reimbursement.
Privacy/Confidentiality
Any/all information written on your health form or verbally discussed in the office, will remain confidential. If anything needs to be shared with other health care professionals, you must first give written permission to do so.
Parking
For your convenience, free parking is available at the office.
Use the driveway to the right of the building and park behind the house.
The entrance is the purple door in the back right corner.
Licensure
Brian Thayer: Massachusetts Massage Therapy License #3179
In
2008, Massachusetts established state licensure for the practice of
Massage Therapy. Any/All therapists practicing Massage Therapy must be
licensed by the state.
